Secret Santa questionnaire and wish list form
Share a few details about yourself. Then copy, download, or print your completed form.
My Wish List
Start with a couple of interests and one gift idea. Empty sections won’t appear in your form.
The template is ready. Edit it to fit you.
You can save the form as a PDF from the print dialog. Share it with the organizer or your Secret Santa. Save it before closing this tab.
Leave Room for a Surprise
Add a few ideas within the budget. “I love plain black tea” is more helpful than “something tasty.”
Secret Santa questionnaire and wish list form
Create a wish list to help your Secret Santa choose a gift. Share your interests, add a few ideas and links to specific items, then edit the form in your own words and send it to the organizer or your Secret Santa.
When to Fill Out a Wish List
A wish list is especially helpful when participants don’t know each other well, such as in a large department or online club. Even friends and family may not know your size, favorite flavor, or what you already own. A few specific ideas can help your Secret Santa choose a gift while keeping it a surprise.
What to Include
Start with your name and a couple of interests. Suggest a few gifts within the agreed budget instead of naming one must-have item. Add sizes, colors, or links if they’ll help. You can also mention what you’d rather not receive.
How to Share Your Wish List
Review the text, then copy or share it, download a text file, or print the form. You can also save it as a PDF from the print dialog. This is a standalone form: it isn’t added to an exchange or sent to your Secret Santa automatically. Share it yourself.
An Example Wish List
Instead of “I like tea,” try: “I like plain black tea; I don’t drink green tea.” Share your interests, suitable gift ideas, and anything you’d rather not receive. You share the form yourself.
